Ex-employer Wants Me To Sign A "voluntary Abandonment Of Claim" After Exposure To A Bat And Rabies Shots In August (i Quit In December)

Hello! I'll try to make it quick. I was employed at my old job in from March 2022 to December 2025. In August of 2025 I was exposed to a bat at work and had to go get four rounds of Rabies vaccines that totaled around $25,000 USD and was billed to the company's insurance under a worker's compensation claim since happened at my place of work. It's been three months since I quit that job. Earlier this week their insurance agent called and asked for screenshots of proof of my ER visits. Today they sent me a message claiming that they confirmed with the state (Colorado) that in order to close my case, I would need to fill out a "Voluntary Abandonment of Claim" file. This seems really fishy and the company is no stranger to shady actions. I left the company on good terms almost four months after it happened but I feel like they're trying to pull something sneaky. Do I really need to sign the abandonment form to be finished with this or are they trying to take advantage of me?
