Trilogy Health Services Acquires Kingston Healthcare, 14 Senior Living Communities

Trilogy Health Services has acquired Kingston HealthCare Company and 14 of its communities in a move that CEO Leigh Ann Barney said is a “great complement” to the existing Trilogy portfolio in Indiana and Ohio. 

The communities deepen Trilogy’s presence in Indiana and Ohio – including in Fort Wayne, where the operator is gaining a new foothold – bringing its total community count nationwide to 144 properties.

The transaction helps Trilogy serve more residents, families and employees, Barney told SHN. Both companies were “closely aligned” on philosophies such as servant leadership and putting care at the center of operations, which made the acquisition a good fit, she added.

“Those values, along with the fit into our current footprint and a strong balance of senior housing and skilled business, made the opportunity very strong,” Barney wrote.

Trilogy in the next 90 days will onboard the new communities and the staffers working in them.

“We want the Kingston employees and residents to feel a seamless transition. We know full integration will take time and we do not want to rush that process,” Barney said via email.

Trilogy did not disclose terms of the deal.

The current complexity of senior living is pushing operators to find new ways to meet resident needs, according to the leader of the Louisville, Kentucky-based operator.that

Looking ahead, Barney said she sees “an exciting 2026” that focuses on “innovation and clinical excellence” as the next generation of senior living residents join communities nationwide.

“We will continue our progress on the clinical side of our business, assuring great personalized care is provided to each resident across our footprint,” Barney said.

In 2024, Trilogy began growing toward a more complete senior living continuum, tackling challenges like rising resident acuity and clinical complexity head-on. To get there, Trilogy and other operators are adopting new technology and AI-supported tools this year to improve clinical care and caregiver support.
